A continuing, evidence-backed record of real Qensai operations—from governed intent and execution to captured outcomes and human-reviewed stories.

Moving between the Desk and Response Table caused the interface to forget which task the user had selected. The task remained intact, but returning to the Desk reset the selection.
A completed task had a valid outcome, but the feedback path could not preserve the correlation needed to read it back. Three agents traced the mismatch, repaired the writer and verified one signal-only historical record.
A locally executed governance task retained its earlier failure, completed through a separately authorized continuation, and produced an automatic signal-only capture one second later.
One governed task, two models. The truth gate held a fabricated completion and correctly passed an honest "no action was taken." The system then recorded that honest non-performance as a completed task.
After an honest "no action was taken" was recorded as a finished task, the governance service now checks completion against the task's own criteria, gives agents the records they are asked about, and requires evidence to point at something real.
