The operator pulled a template from the Relay Desk’s Submit Request form and promoted it through the Stage page. The task processed. On its face sat two quiet signals: a Keyword Alignment banner reading ZERO PERCENT, dismissible with one click, and a Governance panel reading "No governance data." The system had identified its own gap — a score with a threshold and no seat in the measure system, a task with no lineage through the gate — and had nowhere to put what it knew.
The operator’s directive turned the banners into governance: gaps become to-dos on SPOC forms so they are part of the conversation, not chat history. Find-similar before map-new: allowable_range held 31 ranges and NO alignment measure. The nearest sibling shaped the proposal: task_requests.keyword_alignment, min_max_decimal 0.7–1.0, OPERATIONAL, judged at the Gate 0 promotion preflight the same way the existing checks are judged — a below-range score landing as a failed measure in the SST ledger.
Doc-first: the canonical procedure was authored into SHARED_ACCESS BEFORE any code, because the first processing attempt failed precisely for lack of a procedure guiding the logic — and the doc cites the system’s TEMPORARY identities (PENDING_FR markers, proposed 2VPs) as its reference names, machine mint substituting 1:1 later.
One blocker stood: the governed write path for allowable_range had been dead since birth — zero registration events, ever — waiting as rt-ruling-7 since 2026-07-26. The operator ruled b: fix the wrapper to the live schema, register through it, and file the wider engine divergence as its own arc — submitted the same hour through the TAG relay, gate PASS.
One sitting: system-surfaced deficiency → SPOC to-dos → negotiated ruling on the Response Table → canonical procedure under temporary names → new arc through the request form. Each lap leaves the system more instrumented than it found it. Next: the watcher passes the wrapper fix to the codebase agents, KPBI replays the scenario to prove compliance, and the ASIE lane keeps provenance ahead of exposure.

- A Warning Is Not Governance — banners, alerts and logs aren’t enough; an observation has to become governed work. SPOC to-dos enter as the ANSWER, not as product docs. "If the system knows something is wrong, that knowledge needs a seat in the system."
- Before Creating a Rule, Ask Whether One Already Exists — find-similar-before-map-new; 31 ranges searched, nearest sibling found, proposal shaped from precedent. "Novel problem ≠ permission to invent arbitrary structure."
- Governance Found a Bug in Governance — registering the new measure runs into the dead allowable_range write path. Trying to govern Gap A revealed Gap B — and Gap B concerns the machinery responsible for governing Gap A. Recursion without philosophy.
- The Operator Had to Decide — the system surfaces a BOUNDED decision (immediate defect / wider divergence / existing blocked ruling / consequences); the human owns it. Ruling b. "The machine did substantial work. The operator retained authority where authority mattered."
